General Information
Before any athlete can participate in tryouts, practice and /or games, all paperwork and insurance fees are to be collected by the coach.

- OCBE Permission to Participation
- OCBE Alternate Transportation form
- OCBE Drug Testing consent form
- ImPACT Testing form*
- Academic Eligibility
- Parent Communication
- Parent Code of Conduct
* NOTE: The ImPact Testing form is a new form regarding concussions. Although they are titled as high school forms, they are the correct forms for the middle school.
